Disc brake adapters for the 5000 carbon Swing Arm.. who? where?

I am thinking about adding a rear disc to my ProFlex 5000.  I did a search but the topics are years old and the links to the folks selling them are long gone. 

Does anyone have any leads on where I can get one of these coveted disc brake adapters?  I'm guessing you have to drill the swing arm to install the adapter as well. Can anyone confirm or have detailed instructions on the install?  Links?
