How do I mesure my frame?

I want to know what size frame I have, how do I mesure it?  I am assuming there are 2 points to mesure from that everyone goes by.  And what measurement is it what, small, medium, large...

Middle of the bottom bracket to the upper end of the saddle tube.

Meassured in inches or cm.

And what the manufacturers mean by saying S, M, L or something is quite foggy. Everyone has different sizes for S, M, L.
